For your convenience, you may download and print out the new patient form, and the dental and medical history form prior to the initial visit. Please return them by fax at 905-943-9988 or by e-mail at info@centralmarkhamdental.com. If you have dental benefits (insurance plan), please provide your policy and certificate number in advance to your appointment. This information will assist us in helping you to submit claims.
